If you have a clubber, fill out this form. Please bring cash or check (made out to Buckingham Baptist Church) on the first meeting night. 

The Registration Fee for a NEW Clubber is $30.00 for the first child and $20.00 for each additional child. This includes a club shirt or vest and their book. 

If you are a RETURNING clubber and have a club shirt/vest and a book there is no fee. If your clubber is RETURNING and they need a new shirt/vest and/or book, there is a $15 registration fee per item.
